dollar technical audit to determine the cause of operational failures
of several large gas-lift water injection systems. Results were used
in international arbitrations.
- Philadelphia Gas Works: Analysis of an LNG vaporizer
failure with subsequent fire.
- Advanced Aromatics: Investigation of the circumstances
involving a phenol release, with subsequent serious damage to adjacent
equipment and personnel.
- Seagull Pipeline: Investigations regarding fire
& explosion in a natural gas facility.
- Xral: Conducted investigations of the failure
of computer-based process control / SCADA systems that resulted in
multi-million $ product loss from salt dome storage.
- Ramsey County: Assistance in the investigation
of the Williams product pipeline rupture and subsequent catastrophic
fire.
- Warren Petroleum: Analysis of an LPG terminal
accident that resulted in a catastrophic fire.
- Board v. Austin: Investigation of the failure
of a pipeline pig launcher control system.
- Ashland Tank Collapse: Prepared safety review
documents regarding crude oil storage.
- Texaco Port Arthur: Investigation and testimony
regarding the failure of a large catalytic cracking unit that resulted
in multiple loss of life.
- Citgo Lake Charles: Investigation of hot oil /steam
explosion during startup following turnaround.
- Phillips Pasadena: Accident investigation team,
focusing on the control system configurations, subsequent to this
landmark multibillion$ accident.
- Arco Channelview: Primary investigative effort
on this wastewater tank explosion that resulted in multiple deaths.
Significant regulatory liaison was required.
- Milliken: Billion$ destruction of a manufacturing
complex due to failure of a hot oil heat transfer system.
- Shell Norco: Investigation of the catastrophic
failure of the DePropanizer overhead piping.
- Huntsman Polymers: Post accident investigation
of the destruction of a large fired heater.
